PRN - Physical Therapist

Requirements:* * Graduate of an accredited Physical Therapy School. * Licensed in the State of Illinois. * Current CPR card Knowledge of general P.T. practice act. Knowledge of Rehab/PT policy and procedures. * Capable of communicating effectively verbally or in writing. * Must have good leadership skills. *Description:* * Evaluates and records patient treatment plan in coordination with their physician. * Establishes patient treatment programs to include modalities of heat, cold, massage, mobilization, hydrotherapy, light therapy, electricity and patient education as per orders from the physician. * Evaluates gait patterns and instructs patients in proper technique and use of devices. * Coordinates patient care or treatment with other members of the team according to abilities. * Adapts treatments according to the level of improvement of the patient. * Provides home health services through a contract with Addus or Egyptian Health Department. * Performs other duties as required or assigned.
