**Position Overview

**

We are seeking a full-time **Case Manager** with qualifications as a Certified Occupational Therapist Assistant (COTA), Certified Disability Management Specialist (CDMS or CRC), Assistive Technology Specialist (AT Specialist) or other relevant experience working in a disability related field.

The Case Manager will play a vital role in supporting participants with assistive technology needs from program entry through completion. This position requires travel to accessible locations, a reliable vehicle, and car insurance.

**Key Responsibilities**

**Participant Support & Case Management**

- Conduct eligibility screenings and needs assessments to match participants with Assistive Technology that will further their ability to participate in education, employment, and/or community access.

- Collaborate with community partners to identify eligible individuals and ensure seamless service delivery.

- Manage a caseload of eligible participants, providing personalized, participant-centered support.

- Recommend and coordinate tailored AT solutions based on individual needs, goals, and strengths.

- Support coordination for participation in digital literacy training plans for participants.

- Maintain accurate, detailed case records in multiple digital platforms (Microsoft suite software, case management software, mileage applications, etc.).

**Training & Technical Assistance**

- Provide one-on-one training sessions to ensure participants gain proficiency with AT devices.

- Create accessible training guides for AT devices provided to participants.

- Deliver ongoing technical support and follow-up sessions to address challenges and adapt solutions as needed.

**Procurement & Coordination**

- Coordinate the procurement of AT devices with the purchasing department, ensuring timely and accurate order fulfillment.

- Oversee installation of AT devices within the timeline.

**Program Evaluation & Reporting**

- Collect and analyze participant data to assess program impact and effectiveness.

- Administer questionnaires and provide feedback to improve services and outcomes.

- Participate in preparing reports for stakeholders, demonstrating program success.

**Compliance & Collaboration**

- Ensure compliance with federal and state disability laws and ADA guidelines.
