Required Qualifications Occupational Therapist, Rehab Experience: 2 years Licenses: OT-NM Certifications: BLS(Copy needed) SSN Required DOB Required Work History: Should not be more than 30 days of gap in entire work history References: 1 Reference in entire work history -Bachelor's Degree in Occupational Therapy.
