This is a contract position in an outpatient setting. The ideal candidate will have a strong background in physical therapy and a demonstrated history of providing high-quality, patient-centered care in an outpatient setting. This role involves evaluating patients, developing individualized treatment plans, and implementing evidence-based therapeutic interventions to improve mobility, function, and overall quality of life. Duties Conduct detailed evaluations and assessments of patients in an outpatient setting. Develop and implement customized treatment plans based on clinical findings and patient goals. Administer therapeutic interventions such as manual therapy, therapeutic exercise, and functional training. Monitor, reassess, and adjust treatment plans as needed to ensure optimal outcomes. Educate patients and caregivers on treatment strategies, home programs, and preventative care. Collaborate closely with physicians and other healthcare providers to coordinate care. Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards. Uphold professional and ethical standards of physical therapy practice. Qualifications Degree in Physical Therapy from an accredited program. Valid state licensure or eligibility for licensure as a Physical Therapist. Strong knowledge of geriatrics, orthopedics, and acute care practices. Familiarity with systems related to patient care management and discharge planning. Excellent communication skills for effective interaction with patients, families, and healthcare team members. Ability to develop comprehensive care plans tailored to individual patient needs.
