Job Description

Summary

The Cardiopulmonary Rehab Therapist helps to maintain the operations of a comprehensive cardiopulmonary rehabilitation program, to provide high quality patient care in keeping with the standards and strategic objectives as put forth by MyMichigan Health, Rehabilitation Services; under the direction of the CR/PR medical director(s) and/or the CR/PR health system manager.

The Cardiopulmonary Rehab Therapist is responsible for the participation and decisions in daily operations within the cardiopulmonary rehabilitation environment. This includes but not limited to the development, implementation and regulatory compliance associated with patient care and the entire range of services provided in CR/PR such as: patient evaluation/assessment, patient engagement, patient interventions as indicated, patient and community education, exercise training and treatment, psychosocial support and continued reevaluation and follow up in conjunction with the CR/PR program's medical director.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES

(55%)Daily operations and direct patient care consisting of supervised monitored sessions for the cardiac and pulmonary rehabilitation patient, under the supervision of the CR/PR medical director(s). Direct session and patient oversight for all patients utilizing respiratory service codes. These duties may include patient assessments, monitoring patients for physiological changes during session, ECG interpretation, and emergency response to untoward events. The Exercise Physiologist will also be responsible for the development, assessment/re-assessments, modification, and submission of the patient's Individualized Treatment Plan (ITP) to the Medical Director. In conjunction with the ITP the exercise physiologist will assist the patient in maintaining the treatment plan by providing patients with assistance in exercise training, education on health improvement and behavior modification, provide patient with feedback and program outcomes, and communicating any deviations from the plan to the patient's provider.

(15%)Indirect patient care function such as scheduling, insurance verification, billing, EMR work ques, and physician communication.

(15%)In coordination with the education department, Medical Director, and Cardiopulmonary Rehab Manager reviews, maintains, updates Emergency Policies and coordinates training and mock codes for compliance. Will work Cardiovascular Services and/or Respiratory Care teams to educate staff and perform quality improvements measures in patient education and referral process. program certification and national program registry. Leads and/or participates in Health System, Rehabilitation Services, and CR/PR program performance improvement initiatives to maintain the highest level of program quality and patient safety. Actively participates in the Rehabilitation Shared Governance model and the CR/PR Shared Governance Work Groups.

(15%)Maintains patient's chart and performs chart auditing. Performs patient outcome assessments pre/post treatment and as indicated. Performs outcome data collection and interpretation for national program certification and national program registry. Leads and/or participates in Health System, Rehabilitation Services, and CR/PR program performance improvement initiatives to maintain the highest level of program quality and patient safety. Actively participates in the Rehabilitation Shared Governance model and the CR/ PR Shared Governance Work Groups.

OTHER DUTIES AND RESPONSIBILITIES:

Responsible for obtaining: patient history, physical assessment measurements during exercise treatment, such as: exertion, dyspnea, pain, blood pressure, heart rate, oximetry reading, heart sounds, blood glucose levels and Interpretation of ECG rhythms.

Performs patient monitoring for inappropriate responses to exercise, discontinuation of treatment and appropriate reporting, as indicated to the CR/PR supervising physician, medical director and/or the patients referring physician.

Responsible for participation in CR/PR patient emergent situations and the initiation of appropriate emergency responses according to the CR/PR Emergency Procedures and national regulatory guidelines for cardiopulmonary rehabilitation. Will participates in annual emergency preparedness training and mock codes per CR/PR program policies.

Assists in the establishment and maintenance of the student internships and job shadows for the CR/PR multi disciplinary professionals.

Management of Electronic Medical Records (EMR) Work Queues: 1)patient referrals, 2) billing, revenue capture, 3) insurance authorization, 4) patient scheduling and 5) provider and/or intradepartmental communication. 6) Performs all necessary documentation and enters daily assessment data into patient records.

Demonstrates competency in Infection Prevention/Control practices:1) control measures designed to prevent the spread of infectious diseases, 2) implements and maintains isolation techniques, and 3) follows hand hygiene policies and utilizes personal protective equipment as indicated.

Assists with maintenance, calibration, infection prevention and cleaning of CR/PR equipment.

Performs other duties and/or functions as assigned by the CR/PR manager or designee.

Certifications and Licensures

BLS - Basic Life Support

Required Equivalent Experience - Within 1 month of hire or transfer

CARDIAC REHAB THERAP - Cardiac Rehab Therapist - RN or RT

Required Equivalent Experience - Licensed Respiratory Therapist with AACVPR Pulmonary Rehab Certificate within 2 years or Registered Nurse upon hire or transfer. Casual employees are not required to have AACVPR.

Required Education

Education: Associates Degree

Comments: Degree in a healthcare related field. Two years of clinical experience preferred. Any therapist who has RTT credentials prior to July 1, 2004 is not required to have an Associate's degree. If a Registered Nurse, must obtain BSN by 1/1/2025.

EXPERIENCE, TRAINING AND SKILLS:

Knowledge of nursing practices and procedures as normally acquired through 1 year experience working in a Medical Center and similar specialty setting.

Experience in assessment of cardiac and pulmonary patients, the development of individualized exercise prescriptions and individualized treatment plans, and the design of individual exercise regimens for patients receiving treatment in CR/PR.

PHYSICAL/MENTAL REQUIREMENTS AND TYPICAL WORKING CONDITIONS:

Exposure to stressful situations, including those involving public contact, as well as, trauma, grief and death.

Able to wear personal protective equipment that includes late materials or appropriate substitute if required for your position.

Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description. Overall vision and hearing is necessary with or without assisted device(s).

Frequently required to sit/stand/walk for long periods of time.

May require frequent postural changes such as stooping, kneeling or crouching. Some exposure to blood borne pathogens and other potentially infectious material.

Must follow MyMichigan Health bloodborne pathogen and TB testing as required.

Ability to handle multiple tasks, get along with others, working dependently, regular and predictable attendance and ability to stay awake.

Overall dexterity is required including handling, reaching, grasping, fingering and feeling.

May require repetition of these movements on a regular to frequent basis.

Physical Demand Level: Medium. Must be able to occasionally (0 33%of the workday) lift or carry 21 50 lbs., frequently 34 66% of the workday 20 lbs. and or Walk/Stand/Push/Pull of Arm/Leg controls and constantly (67 100% or the workday) 10 lbs.
