Position Overview: The ideal candidate will provide high-quality physical therapy services primarily in an outpatient therapy center, with additional opportunities in inpatient and home health settings. This position offers flexibility in scheduling, allowing you to choose between a 4-day or 5-day workweek, while weekend coverage is rotated among the PT team. Key Responsibilities: Evaluate, develop, and implement personalized treatment plans for patients across a range of settings, including outpatient, inpatient, and home health. Collaborate closely with a multidisciplinary team to deliver comprehensive patient care. Educate patients and their families on treatment plans, exercise routines, and home care strategies. Maintain accurate and up-to-date patient records, adhering to all regulatory and compliance standards. Participate in team meetings, case conferences, and continuing education opportunities to ensure best practices in patient care. Qualifications: Doctor of Physical Therapy (DPT) or a Master's degree in Physical Therapy from an accredited program. Current state licensure or eligibility for licensure as a Physical Therapist in Illinois. Strong interpersonal skills and a commitment to delivering high-quality, patient-centered care. Compensation and Benefits: Competitive Salary: Up to $86,200 - $104,200 per year, depending on experience.
