Physical Therapy Assistant - Site Coordinator

Lakeview Village is searching for a Physical Therapist Assistant to join our team as a site coordinator!

Lakeview Village in Lenexa, Kansas is a not-for-profit continuing care retirement community that offers a variety of living options and services to seniors, including Outpatient Therapy. In addition to an on-campus outpatient therapy clinic, Lakeview's therapy team also provides services at six other locations near the main campus. Must be a Licensed Physical Therapy Assistant in the State of Kansas.

The site coordinator performs duties related to the scheduling and coordination of restorative programs for Care Center residents, assists in the treatment of residents as directed by the Physical Therapist in order to restore, maintain, or prevent the decline of resident function, and participates as an active member of the rehabilitation team.

Schedule: Monday-Friday 8:30a-5:00p

Pay Range: $26 - $38/hr, based on experience

Job Duties and Responsibilities:

• Performs scheduling duties for Eastside Terrace/Care Center residents, working closely with the Eastside Terrace and Care Center managers.
• Coordinates the scheduling of tasks with the designated Outpatient and Home Health schedulers.
• Communicates with the scheduling team through the Rehab scheduling email group. Consults with managers regarding staffing decision as needed.
• Receives all staffing requests from managers and staff related to staff availability and the fulfillment of resident care needs. Ensures that hours are distributed equally.
• Assist the Eastside Terrace Therapy Supervisor in proper allocation of therapy minutes based on the guidelines from CMS and managed care entities.
• Assists in gathering information for team and family meetings, attends meetings as required.
• Assists in gathering and sending documentation for review as directed by the Eastside Terrace Therapy Supervisor.
• Assists in procuring durable medical equipment for residents in Eastside Terrace Rehab.
• Provides treatment to residents under the direction of the Physical Therapist.
• Records treatments administered in residents' medical records and in the department's daily billing records, in accordance with established procedures.

Qualifications

• Must be a graduate of an accredited Physical Therapist Assistant program and licensed to practice in the State of Kansas.
• Previous experience working with the elderly is preferred.
• Demonstrates good communications skills, including but not limited to effective reading, writing, speaking, and analytical skills, with a fluent understanding of English.
• Possesses an empathetic approach to the elderly.
• Able to work in a team setting with residents, relatives, staff, and others.
