Bill Rate: $63/hr + IRS Mileage Rate

Schedule: Monday-Friday
Guaranteed Work Week: 40
Coverage: Mesquite, Terrell, Kaufman, Crandal
May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address.
Average 4-6 visits per day depending on type of visit for PT/OT/SLP
Productivity is 28-32 points per week for PT/OT/SLP
SOC: 2.5 points, ROC: 2 points, Evaluations/Recert/Discharge OASIS: 1.5 points, Discipline DC, routine visit, 30-day reassessment: 1 point, Weekly case conference: 1 point
Average 6-7 visits per day depending on type of visit for PTA/COTA
Any overtime must be pre-approved.
2 weeks cancellation
Home Health and Oasis experience required, no exceptions
