Job Description:

Are you a compassionate and experienced Physical Therapist looking to make a difference in the lives of patients in the comfort of their own homes? We are seeking a skilled Home Health Physical Therapist to join our team and provide exceptional care to our patients in Gretna on a 13-week contract.

As a Home Health Physical Therapist, you will play a vital role in assessing and developing treatment plans for patients who require physical therapy services in a home health setting. You will collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists, to optimize patient outcomes and promote their overall well-being.

Responsibilities:

  • Conduct comprehensive initial assessments to evaluate patients' physical conditions, functional abilities, and mobility limitations.
  • Develop individualized treatment plans based on assessments, patient goals, and physician orders.
  • Administer therapeutic exercises, manual therapy, and modalities to improve patients' strength, range of motion, and functional independence.
  • Educate patients and their families on proper techniques for exercises, transfers, and use of assistive devices.
  • Monitor and document patients' progress, modifying treatment plans as necessary.
  • Communicate regularly with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure treatment goals are met.
  • Maintain accurate and timely electronic medical records and submit required documentation in accordance with regulatory guidelines.
  • Adhere to ethical and professional standards, as well as compliance with state and federal regulations.

Requirements:

  • Bachelor's or Master's degree in Physical Therapy from an accredited institution.
  • Current state license or eligibility for licensure as a Physical Therapist.
  • Experiences in a home health setting.
  • Proficiency in utilizing HomeCare HomeBase (HCHB) electronic medical record software.
  • Strong clinical and critical thinking skills, with the ability to assess and modify treatment plans based on patient progress.
  • Excellent communication and interpersonal skills to collaborate effectively with patients, families, and healthcare team members.
  • Compassionate and patient-centered approach to care, with the ability to motivate and empower patients to achieve their therapy goals.
  • Understanding of Medicare regulations and documentation requirements for home health services.
  • Valid driver's license and reliable transportation to travel to patients' homes.
