JOB DESCRIPTION

HOME HEALTH SPEECH / LANGUAGE PATHOLOGIST

Department: WCAH Home Health

Occupational Summary:

Provide speech / language pathology and audiology services and ongoing assessment and evaluation of functioning for skilled patients. Provide education and consultation with the patient/family and other organizational personnel. Accurately document evaluation and treatments provided.

DUTIES AND RESPONSIBILITIES

  1. Provide services in accordance with accepted standards of practice by or under the direction of a qualified Speech-Language Pathologist (SLP) or Audiologist.

  2. Provide initial and ongoing assessments to identify the client's level of functioning and ability to communicate, including OASIS assessments at appropriate time points

  3. Develop the plan of care in consultation with the physician and other care team members, and revising it as appropriate

  4. Prepare clinical and progress notes

  5. Consult and coordinate services with other disciplines

  6. Evaluate effectiveness and outcomes of care.

  7. Plan for discharge

  8. Participate in in-service programs.

EDUCATION , TRAINING & EXPERIENCE

Holds a master's or doctoral degree in speech language pathology, and

Meets the education and experience requirements for a Certificate of Clinical Competence in audiology granted by the American Speech-Language-Hearing Association; or

Meets the educational requirements for certification and are in the process of accumulating the supervised experience required for certification.

RELATIONSHIPS

Reports to the Therapy Team Leader

REQUIREMENTS/QUALIFICATIONS

  1. Must possess good verbal and written communication skills

  2. Must be able to meet deadlines, work under limited supervision.

  3. Climb at least 2 flights of stairs and be able to stand and/or walk for a period of at least 4 hours

  4. Valid state driver's license

  5. Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers.

  6. Proof of motor vehicle insurance
