S*eeking to hire a Physical Therapist to deliver care to patients in their place of residence. You will help people fully engage in their lives by providing exercises, strategies, adaptations, adjustments, and tools that help them function more independently.**No two days are the same.** Every patient has different needs and challenges, and every residence provides a change of scenery. You'll be following the plan of care and providing services that help reach goals set forth by their physician.**Duties:**- Provide a variety of physical therapy services to clients in their residence in accordance with physician's authorized plan of care- Determine each patient's functional status and functional limitations in regards to activities of daily living.- When a physical therapy or certified home health aide is assigned, prepare written exercise program; supervises, instructs and directs in accordance with regulatory guidelines, Agency and accreditation standards- Determine patient's equipment needs- Evaluate and regularly re-evaluate the physical therapy needs of the patient, and assess their continual care needs- Initiate diagnostic, preventive, and rehabilitative procedures as appropriate, and make referrals to other disciplines as indicated by the needs of the patient.- Observe signs and symptoms and report changes in the patient's condition to the appropriate health professionals as needed- Develop, prepare, and maintain individualized patient care progress records and submit documentation within 48 hours of visit- Keep current in physical therapy trends and knowledge and participate in in-service programs**Qualifications:**License to practice as a Physical Therapist in the State of MichiganHas graduated from a physical therapy curriculum approved by:The American Physical Therapy Association, orThe Committee on Allied Health Education and Accreditation of the American Medical Association, orThe Council on Medical Education of the American Medical Association and the American Physical Therapy Association; orGraduated prior to January 1, 1966:Was admitted to membership by the American Physical Therapy Association, orWas admitted to registration by the American Registry of Physical Therapist, orHas graduated from a physical therapy curriculum in a 4-year college or university approved by a State department of education; orHas two years of appropriate experience as a physical therapist, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking qualification as a physical therapist after December 31,1977; orWas licensed or registered prior to January 1, 1966, and prior to January 1, 1970, had 15 years of full-time experience in the treatment of illness or injury through the practice of physical therapy in which services were rendered under the order and direction of attending and referring Doctor of Medicine or osteopathy; or
