Job Summary

The Respiratory Therapist (RT) Navigator/Lead, also known as the Pulmonary Navigator, plays a vital role in bridging clinical care and patient support for individuals with complex pulmonary conditions. Serving as a dedicated care coordinator, the RT Navigator guides patients through every stage of their treatment journey-from diagnosis to long-term management-ensuring they receive comprehensive education, advocacy, and access to essential resources. This role involves direct and indirect patient care in an outpatient setting, including advanced disease care coordination, post-discharge follow-up, facilitating referrals, data collection and analysis, and community outreach. By acting as a liaison between patients, families, and healthcare providers, the RT Navigator helps improve patient outcomes and quality of life through personalized, compassionate support.

Essential Functions and Responsibilities

  • Provides direct support to the non-invasive ventilation clinic (NIV Clinic).
  • Assists in development and direct support of the interstitial lung disease (ILD) program.
  • Provides patients and their families with comprehensive information about their pulmonary condition, treatment options, and self-management strategies.
  • Works with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans.
  • Facilitates referrals for pulmonary rehabilitation, testing, and other necessary services, as well as coordinating follow-up care.
  • Serves as a patient advocate, addressing concerns, and ensuring patients receive necessary resources and support.
  • Tracks patient outcomes, identifies areas for improvement, and contributes to quality improvement initiatives.
  • Engages with the community to raise awareness about pulmonary diseases and promote early diagnosis and treatment.
  • Works closely with inpatient COPD navigators.
  • Perform additional duties and/or separate responsibilities as assigned.

Education and Experience

  • Graduate of an Accredited College/University Respiratory Therapy School.
  • Minimum of five years' experience in Respiratory Care. Sleep experience preferred but not required.

Certifications, Licenses, Registrations

  • Registered Respiratory Therapist (RRT) credential through the National Board of Respiratory Care is required.
  • Must have current State Respiratory Care Practitioners license for states you will provide service in.
  • Basic Life Support (BLS) certification required and must recertify at least 2 weeks before expiration date
