As a Case Manager for Foster Care, you will provide case management and comprehensive community support services to children and their families. More specifically, the candidate chosen for this role will: Maintain regular, ongoing contact with each child/family on caseload to ensure coordination and provision of needed services in compliance with the client's plan of care. Contacts will occur face-to-face or via telehealth according to licensing regulations and identified need. Frequency/type of contact will be dictated by plan of care and program guidelines for which the client is enrolled. Deliver case management services aimed at on-going assessment of the client's need for medical, educational, social or other services and ensures coordination/provision of these services as indicated in the client's plan of care. Engage in case management activities which may include making referrals for identified services, scheduling appointments, collaborating and maintaining communication with service providers, providing consultation and support to parents/legal guardians on an on-going basis and face-to-face monitoring/contacts with the client/family as often as necessary or at a minimum frequency as outlined in program guidelines. Provide comprehensive community support services for identified clients that will consist of activities needed to allow the client to live with maximum independence in the community through the use of skills training as identified in the client's plan of care. Initiates and maintains regular contact with other service providers involved with the child (i.e., school, therapists, social workers, medical professionals, etc.) and informs and involves other team members in the treatment planning process. Participates in/responsible for the treatment team meeting and the development of the specific care plan. Responsible for completing required documentation within designated time frames and in compliance with reimbursement standards outlined by the Department of Medicaid Services or other payors. Actively plans for the effective transition of the client from services through on-going discharge/termination planning and referral for services still in need of attention at the time of termination, if indicated. Provide supervision of therapeutic foster homes for clients in therapeutic foster care program as assigned. Responsible for assuring completion, and obtaining all required documentation by the foster family. Provide foster families at least weekly contact by phone and has two face-to-face contacts monthly with the family and one individual visit with the youth outside of the home monthly. Evaluates foster families annually for the process of re-certification of their homes, completes documentation, and provides documentation for staff responsible for annual recertification process. Assist families in implementing the in-home strategies specified in the treatment plan. Provide 24 hour on-call coverage, on a rotating basis, to families/clients in the event of crisis situations. Assist prospective foster/adoptive families in completing all requirements for certification/re-certification. Including participating in the pre-service training schedule as needed. Receives referrals, responds to referrals in a timely manner, documents referral status, communicates with families to coordinate matches, and coordinates placement of children into therapeutic foster/adoptive homes as assigned. Education and/or Experience: Bachelor's degree from an accredited university in social sciences or related field. One (1) year of full-time experience working with children in a human service setting. Certificates, Licenses, & Registrations Must successfully complete a department approved targeted case management training within six (6) months of employment and successfully complete department approved continuing education requirements every three (3) years thereafter. Certified and maintains updates in CPR, and First Aid. Completion of safe driver's training course through agency with valid Driver's license and insurable driving record required.
