Substance Use Disorder Community Case Manager - $2,000 Sign On Bonus

As the SUD CCM some of your duties will be to provide support with a recovery focus to adult patients experiencing co-occurring mental health and substance use disorders and to assist patients to discover and maintain a valued role(s) in the community. You will complete case management assessments and develop comprehensive care plans and individual treatment plans and monitors services received. You will assist patients to maintain community living by teaching activities of daily living and personal care; promotes independent and productive habits to patients in the community. Qualifications: Bachelor's degree in a Human Services Field preferred but not required. One to two years of experience with emphasis in mental health preferred, or lived experience. Must be a Certified Recovery Support Worker (CRSW) or eligible to become a CRSW in NH.
