MedAdventures is currently hiring an Physical Therapist (PT) for a contract-to-hire opportunity in Albuquerque, NM. This is in an outpatient setting. It's an excellent opportunity for a PT passionate about working with the aging population in an interdisciplinary team environment.

Duties

• Screen and evaluate participants for rehab needs at acute, restorative, and maintenance levels
• Deliver physical therapy treatments, including modalities, therapeutic exercise, and mobility training
• Make home visits and provide caregiver education as needed
• Communicate effectively with interdisciplinary team, families, and caregivers
• Delegate treatment plans to PTAs and Rehab Aides as appropriate
• Coordinate with OT and ST services and refer to medical clinic when necessary
• Ensure appropriate use, maintenance, and documentation of durable medical equipment (DME)
• Participate in peer reviews, staff training, and interdisciplinary planning
• Maintain accurate documentation and contribute to quality management activities

Required:

• Bachelor&s degree in Physical Therapy
• Graduate of an APTA-accredited PT program
• Active PT license in New Mexico
• 1+ year of experience working with elderly or frail populations

Preferred:

• 2+ years of outpatient or PACE experience
• MSPT or DPT
• Bilingual a plus
