Occupational Therapist - Outpatient Rehab Services

$34.82 - $52.24 an hour - Full-Time Days, 40 hours per week
Boulder Community Health is seeking an Occupational Therapist who will provide occupational therapy services to a variety of outpatients, meeting departmental productivity standards. Services include evaluation, treatment, re-evaluation, education, discharge planning, goal setting, documentation, communication, reporting, etc., in relation to patient care.

Qualifications
•BS or MA/MS degree in occupational Therapy program as required by Colorado licensure or registration.
•Current registration as an OT with the State of Colorado.
•BLS Certification required by the end of the conditional period.
Responsibilities
•Documents legible patient information in the chart.
•Transfers patients from wheelchair, bed, mat, commode, etc.
•Read patients' charts.
•Communicates with patients and their families, staff, physicians, etc., utilizing written and verbal communication.
•Works with patients in a variety of positions (kneeling, sitting, standing, bending).
•Positions a patient properly in their wheelchair (adjusting leg rests, etc., as needed).
•Selects and performs appropriate evaluations to determine the patient’s course of treatment.
•Formulates an individualized patient plan of care.
•Applies appropriate therapeutic procedures and modalities.
•Educates patients and their families.
•Evaluates patient response to treatment and revises plan of care.
•Provides treatment using appropriate tools.
•Plans patient discharge (ordering equipment, recommending therapy, etc.).
