JOB SUMMARY
The RN Case Manager - Rehab Liaison identifies adult and pediatric rehab candidates who meet program criteria and who will benefit from program services by completing a thorough assessment of candidates including- chart review, clinical assessment, and interaction with the ancillary staff and family directly involved in the candidates care. This individual works collaboratively with the patient care team to coordinate a smooth transition from the referring facility, or site to the rehabilitation setting, or will provide recommendations to candidates who are denied admission. After admission, the RN Case Manager - Rehab Liaison is responsible for collecting, comparing, collating, entering, and processing the information required for facilitating payment of patient stays on Rehabilitation. This individual serves as part of the collaborative care team, addressing the patients plan of care and promoting patient-centered outcomes that will encourage the highest possible level of independence and quality of life. The RN Case Manager - Rehab Liaison serves as a resource to the care team with regards to discharge planning and provision of quality and cost-effective health and social care services.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: Graduate of an accredited school of nursing.
Preferred/Optional: None
EXPERIENCE
Minimum Required: One year of clinical experience in rehabilitation or related field.
Preferred/Optional: RN with acute inpatient rehabilitation experience is or equivalent experience in acute care rehabilitation.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required:
- Current State of Wisconsin Registered Nurse License or Nurse Compact License.
- Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire.
Preferred/Optional: Certification in rehabilitation nursing or related specialty is highly recommended. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
