Pay is commensurate with experience within a range of $20.00 to $27.86 per hour. Full time, non-exempt, benefits eligible. Responsibilities: The primaryresponsibilities of the Residential Case Manager will be to support theresidential housing team by providing specialized housing services that includethe operation of the Transitions (Tier II) housing facility, supportive housingcase management, crisis intervention, psychosocial groups and other supportservices to adults that struggle with recurrent mental health disorders alongwith issues in acquiring and maintaining independent community housing. Specific duties include but not limited to:

  • Provide support to consumers to reduce theeffect of those symptoms within a supportive, 24/7 residential environment;
  • Completestrength assessments, collaborate with the residents in their treatmentplanning along with the review of their individualized treatment plan;
  • Assistclients in learning and practicing news skills that contribute to theirrecovery with a goal to achieve independent community tenure as specified intheir individualized treatment plan;
  • Coordinatethe client's services with other providers, agencies, family and communitypartners;
  • Deliveringreimbursable hours of service compliant with established BNC standards;
  • Additionalresponsibilities that may be assigned as circumstances warrant;
  • Flexibilityto provide some after hour assistance to clients in accessing other servicesand/or providing one on one crisis interventions;
  • Regularand predictable attendance is an essential function of the job.

Qualifications : The Supportive Housing Case Manager positionrequires but not limited to:

  • BA/BS degree (psychology,social work, or psychiatric nursing preferred) and/or four years of equivalent experience working in the humanservices field (supportive housing or residential services preferred;
  • Must be able to lift 25 lbs or more to assist theclient in ADLs, IADLs;
  • Must have reliable transportation and valid,unrestricted driver's license;
  • Strong oral and written communication skills;
  • An ability to work independently and in cooperationwith others;
  • Commitment in providing services with a TraumaInformed and Person-Centered approach;
  • Also brings an energetic and positive approach to therapidly evolving changes within the housing program and the challenges of acomplex workplace;
  • Flexibility in assigned working hours.
