Case Manager
Job Description

The Case Manager is responsible for building strong communication and relationships with clients and their collateral partners, including schools, funding partners, therapists, agencies, clinicians, supervisors, and foster homes. This role involves setting limits for youth, developing interventions, and setting goals for individual success in collaboration with the client's guardians, therapists, and other providers. The Case Manager will initiate and monitor external recommendation processes and treatment planning for their caseload, ensuring compliance with policies and procedures, including all contract sources. They will also interact effectively and report timely for computer reports, tracking, and paper reports according to agency protocols. As needed, participation in on-call services is required. Furthermore, the Case Manager will represent the agency on task forces and community boards regarding interests and possess strong interpersonal skills throughout all levels of the agency. Attendance at meetings across the agency and with collateral partners is expected, along with staying informed about trends and significant events in the field of Human Services and Case Management.

Responsibilities

Work Environment

The work environment includes a typical office setting with common areas and offices, where the noise level is generally low to moderate. Additionally, work may be conducted in individual homes with environments unknown to the agency. The schedule is Monday to Friday, 9am-5pm, with potential for extended hours or weekend work if needed. Dress code is business casual. Benefits include tuition reimbursement, starting with 2 weeks of PTO, 12 paid holidays, up to 5 weeks PTO after 10 years, and 55 cent mileage reimbursement.

Pay and Benefits

The pay range for this position is $49920.00 - $52000.00/yr.
