Job Description
We are seeking a dedicated Physical Therapist to join our healthcare team in Minnetonka. As a vital member of our rehabilitation services, you will provide comprehensive physical therapy care while working collaboratively with our multidisciplinary team to deliver optimal patient outcomes.
- Conduct thorough patient evaluations and develop individualized treatment plans
- Implement evidence-based physical therapy interventions and monitor patient progress
- Document patient assessments, treatment plans, and progress notes with attention to detail
- Supervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians
- Establish measurable treatment goals and adjust plans based on patient progress
- Provide patient and caregiver education regarding treatment plans and home exercise programs
- Collaborate with referring physicians and other healthcare providers
- Maintain accurate and timely documentation in compliance with healthcare standards
- Utilize rehabilitation equipment and techniques effectively
- Participate in regular team meetings and care coordination discussions
Qualifications
- Graduate degree from a CAPTE-accredited Physical Therapy program
- Current Minnesota state physical therapy license or eligibility to obtain one
- Valid CPR certification
- Strong knowledge of anatomy, physiology, and rehabilitation principles
- Excellent assessment and diagnostic skills
- Proven ability to develop and implement effective treatment plans
- Strong documentation and electronic health record skills
- Outstanding interpersonal and communication abilities
- Physical stamina and ability to assist patients with mobility
- Problem-solving and critical thinking skills
- Ability to work independently and as part of a healthcare team
- Commitment to continuing education and professional development
- Experience with diverse patient populations preferred
