This is performed in accordance with physician or advanced practice practitioner's orders and plan of care under the direction and supervision of the Home Health leadership team. *In this position you will be required to: *- Evaluate patient's functional status including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.- Evaluates home environment for hazards and barriers to more independent living- Participates in the development of the individualized plan of care including equipment needs. - Develops treatment program and establishes goals for improved function- Communicates plan of care to referring physician and other organization professionals.- Teaches new skills or retrains patients in once familiar daily activities that have been lost due to illness or injury, in accordance with the agency's policy- Maintains appropriate clinical records, progress notes, and reports to the physician or advanced practice practitioner any changes in the patient's condition. Submits records in accordance with the agency's policy**Required Qualifications**A graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association1-year clinical experience as an Occupational Therapist preferredRegistered with the National Registration Examination of the American Occupational Therapy AssociationRequires interpersonal skills and ability to communicate professionally and effectively with staff, patients, and caregiversKnowledge of medical terminology requiredAble to demonstrate problem-solving and conflict resolution skills, organizational skills, and attention to detailMust have strong verbal and written communication skillsDemonstrates ability to handle multiple priorities, documentation requirements and deadlines.Reliable transportation and valid automobile insurance coverageSit, stand, bend, l
