Mental Health Cooperative, Inc. (MHC) was formed in 1993 to serve individuals with severe and persistent mental illness. Since then, we have expanded our services to children and adolescents with severe emotional disorders across Middle and East Tennessee.
Our sole purpose is to support and treat those challenged with serious mental illness and poverty. Although based out of Nashville, we serve several communities across middle and East Tennessee with satellite offices in Antioch, Gallatin, Dickson, Columbia, Cleveland, Murfreesboro, Clarksville, Cookeville, Chattanooga, and Memphis.

Physical Therapy on Flourish Health Clinic
Key Responsibilities:
Flourish Health Clinical Service Delivery
Licensed physical therapist will deliver trauma-informed physical therapy services, helping patients manage pain, improve mobility, and regain functional independence through on-site physical therapy assessments, treatment plans, and rehabilitative care. Services include, but are not limited to:
- Perform musculoskeletal and neuromuscular assessments
- Develop and implement individualized treatment plans tailored to patients with limited access to follow-up care or rehabilitation facilities
- Provide hands-on therapy techniques such as manual therapy, stretching, and therapeutic exercises
- Instruct patients in home exercise programs using easily accessible equipment or bodyweight techniques
- Address acute and chronic conditions such as back pain, joint dysfunction, gait instability, and post-operative rehabilitation
- Fit and educate patients on proper use of assistive devices (e.g., canes, walkers, braces)
- Document patient interactions and progress clearly and securely, in coordination with the clinics electronic health record system
- Collaborate with physicians, nurses, case managers, and social workers to support whole-person care and identify referral options when needed
- Educate patients on body mechanics, injury prevention, and strategies for maintaining physical health despite environmental challenges
