Model the delivery of interactive mental health services in the community to adults designated as having significant mental health challenges. The goal of service delivery is to provide a normalizing experience by enhancing overall functioning while maintaining the youth/adult in the least restrictive environment of their home and community.
Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
ESSENTIAL FUNCTIONS:
Supervise all CSS Adult Case Management Staff
Track staff effectiveness and efficiencies on a month to month basis; while working with staff who are struggling to meet expectations to improve.
Develop performance improvement plans for staff to be able to follow a course of action for correction.
Work with CSS Clinical Manager to identify when new positions are necessary, interview for those positions, hire new staff and help onboard new staff.
Responsible for hiring, development, correction and firing of Case Management staff.
Deliver intensive, community-based, interactive mental health services to adults utilizing a strengths-based, skill-building philosophy
Participate in the development and maintenance of a client-centered Treatment Plan along with the family and treatment team
Work with the CSS Management team for the overall development, improvement and growth of the CSS Department.
Be willing to provide case management or crisis case management services to any Horizons client
Assist the adult in developing a basic understanding of their symptoms and possible solutions
Assist the adult, and/or family in enlisting natural community supports in developing solutions
Network with resource community based organizations to guide new or inexperienced staff in services available- including but not limited to SFCS, Youth Shelter, court system, schools, job opportunity sites , housing agencies, DCF, and Intake & Assessment
Build and maintain relationships with vital community partners (NAMI, Housing Authority, Salvation Army, First Call, etc)
MINIMUM KNOWLEDGE AND SKILLS REQUIRED:
Proficient in electronic documentation, good computer proficiency
Experience working with educating and teaching clients about self care, improving daily living skills
Strong organizational skills
Good time management skills
Be an active listener, positive mentor/role model
PREFERRED BEHAVIORAL SKILLS:
- Research additional services/interventions to encourage community support systems
MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
- One of the following is required:
- Bachelor's degree, preferably in Social Work, Psychology, or some related field
OR
- Associate's degree with an additional 2 or more years' experience working with educating and teaching clients about self-care, improving daily living activities
OR
Total combined years of schooling and experience (working with educating and teaching clients about self-care, improving daily living activities) to equal 4 or more years.
Must have at least one years' experience in Community Mental Health Case Management with knowledge of the treatment planning process, information regarding community resources available, HIPAA boundaries, CSS protocol, progress notes, additional paperwork & HMHC guidelines/rules.
Proven ability to manage caseloads, organize and structure time well, prioritize and triage tasks, and meet the performance requirements of a Case Manager.
PREFERRED EDUCATION AND EXPERIENCE:
Two years' experience in Community Mental Health Case Management with knowledge of treatment planning process, information regarding community resources available, HIPAA boundaries, CSS protocol, progress notes, additional paperwork & HMHC guidelines/rules.
Degree in Psychology, Social Work or other Human Services field.
Three to five years' professional experience in mental health services or related field.
LICENSURE OR REGISTRATION REQUIRED:
Must have completed CPST Case Management on-line training, KHS on-line training, passed KBI, DCF child abuse check, adult abuse registry, and motor vehicle screens; and must have completed Adult Case Management training.
Valid driver's license, insurance and a dependable vehicle
