LICENSED PHYSICAL THERAPIST Direct HireMonday - Friday8:00 am - 5:00 pm$40.00 - $50.00 per hour, DOE ESSENTIAL JOB FUNCTIONS Provide direct patient care in a home health setting.Evaluate patient functional needs and develop individualized treatment plans.Supervise and collaborate with Physical Therapy Assistants.Implement care plans and adjust based on patient progress.Document assessments, goals, progress notes, and discharge plans.Educate patients and families on home exercise programs.Communicate effectively with care teams to ensure optimal outcomes.Maintain compliance with safety, infection control, and quality standards.Promote community awareness of physical therapy services.Participate in ongoing professional development and staff education. JOB REQUIREMENTS Graduate of an APTA-accredited Physical Therapy program.Current license from the Texas State Board of Physical Therapy Examiners.Home health and geriatric care experience preferred.Ability to lift, position, and transfer patients.Requires frequent standing, walking, bending, and kneeling.Strong interpersonal, communication, and documentation skills.Ability to care for patients across a variety of age groups and physical needs.
