Physical Therapist-Must have current Physical Therapist experience.

Setting : Geriatrics

Location : Loveland, CO

Company : MedAdventures- Contract-to-hire and Contract Candidates will be considered.

Hours : Mon-Fri 8am-4:30pm

10% Travel to participants homes and to cover at other facilities as needed. Home health visits will be in Ft Collins, Loveland, Greely. HH experience is not required as this is not Medicare. Must have adult or geriatric experience. 10-25% of their time in homes, 75-90% of their time in the center.

Job Description:

Join our interdisciplinary team providing clinical physical therapy for adults 55+ in a dynamic, supportive environment. You'll evaluate participants' needs, develop individualized treatment plans, and collaborate across disciplines to ensure high-quality, holistic care.

Key Responsibilities:

• Evaluate and treat participants for acute, restorative, or maintenance needs
• Develop and integrate PT plans into the overall Plan of Care (POC)
• Collaborate with caregivers, nursing home staff, and interdisciplinary team members
• Educate participants and caregivers on mobility, DME, and safety
• Supervise PTAs and Rehab Aides per scope and complexity of care
• Document care and participate in quality management activities

Requirements:

• Bachelor's in Physical Therapy and active state PT license
• Accredited PT program completion
• At least 1 year of experience with the elderly or frail population

Preferred:

• 2+ years of PT experience
• MSPT or DPT
• Bilingual skills a plus

Contract-to-hire and Contract Candidates will be considered.
