Harbor is seeking a Case Manager to join the Port Sylvania team! This position is responsible for the case management activities of clients with substance abuse and behavior health problems, as well as assisting clients in obtaining the necessary living/recovery skills to achieve their highest potential.

Position is full-time, 8AM - 4PM.

Education/Experience/Other Requirements:

  • Minimum of a High School Diploma and 3 years of experience in a behavioral health or related field required. Associate's Degree in Counseling/Social Work/ Related Field or Bachelor's degree in human service field or equivalent experience preferred.
  • Chemical Dependency licensure required within 90 days of employment.
  • Knowledge of 12-step groups of recovery.
  • Must have a valid driver's license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor's vehicle insurance carrier.
  • For residential programs; Must be at least 23 years of age, have reliable transportation, must remain up to date on annual Defensive Driving for 15-Passenger Vans training requirement.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
  • Must be proficient and accurate in computer use, including Microsoft Outlook and Word.

Essential Job Competencies/Primary Duties:

  • Schedule and meet with each client as needed, develop treatment plan, and schedule aftercare appointments.
  • Organizes, schedules, and coordinates all case management activities including transportation to appointments.
  • Conducts groups and individual services to provide education about recovery and to support clients implement their Relapse Prevention Plan.
  • Completes all appropriate paperwork for client charts.
  • Understands and maintains professional boundaries in client interactions.
  • Participates in interdisciplinary treatment planning.
  • Networks with outside agencies, as needed, to ensure individual client needs and goals are met.
  • Presents workshops to other agencies on women's specific recovery issues, as needed.
  • Participates in training and development programs to enhance knowledge, skills, and personal service network.
