Home Health - Full Time HHA Physical Therapist
Hill Country Preferred Senior Care
Location: Junction, TX
Job Summary:
The Home Heal - Physical Therapist will be responsible for providing physical therapy services to patients in their homes. The Physical Therapist will work with patients to develop treatment plans, monitor progress, and adjust treatment as needed. The Physical Therapist will also work closely with other healthcare professionals to ensure that patients receive comprehensive care.
Responsibilities
1. Conducts patient assessments and assists the physician in
evaluation the level of function by applying diagnostic and
prognostic functional ability tests.
2. Assist in the development of the Plan of Care.
3. Directs and aids patients in active and passive exercise, muscle
re- education; as well as gait, functional, ADL, transfer, and
prosthetic training.
4. Treats patients to relieve pain; develop and restore function; and
maintain maximum functioning using means such as exercise, massage,
heat, water, light and electricity.
5. Makes use of ultra violet and infrared lamps, diathermy,
ultrasound, whirlpool, contract baths, and applies moist packs.
6. Observes, records, and reports to the physician the patient's
response to treatment and changes in the patient's condition.
7. Instructs patient, significant others, and staff in the care and
use of wheelchairs, prosthetic, and orthopedic devices; and in
areas of physical therapy in which they can participate and assist
the patient.
8. Visits patient according to Plan of Treatment; completes a progress
note for each visit; and submits progress notes to the agency on an
at least weekly basis.
9. Participates in staff conferences and committees as necessary.
10.Attends all education and in services as required.
11.Prepares a written discharge summary and a written summary report
on patient's condition at least every 60 days.
Job Requirements:
1. Current state license as a Physical
Therapist
2. Graduate of an American Physical
Therapy Association approved physical
therapy program.
3. Minimum of 1 year of experience in home health
4. Strong communication and interpersonal skills
5. Ability to work independently and as part of a team
6. Excellent time management and organizational skills
7. Current driver's license; with current automobile insurance.
