The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use disorders, intellectual/developmental disabilities, and housing and homelessness. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community. Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. The Case Manager III provides case management services (assessment, treatment planning, linking, monitoring, and advocacy) to an assigned caseload of children and adolescents with serious emotional disturbance (SED). Completes clinical documentation in accordance with agency guidelines. Department Hiring Salary Range: $46,589-$53,577 Essential Functions Essential functions include but are not limited to:
Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
Preferred Experience:
At least two years of experience in behavioral health care.

- Provide a safe and welcoming environment while providing care coordination services to consumers and families.
- Serves as a primary point of contact for consumers and families being served by the clinic.
- Coordinate care, working collaboratively with prescribers, counselors, nurses, and administrative staff.
- Links consumers to resources, internal as well as external, and provides ad hoc assistance with navigating the service system.
- Facilitates communication and coordinates care between CSB providers and consumer's external healthcare providers for continuity of care.
- Assists consumers and families with accessing behavioral health crisis services when needed.
- Advocates for consumers and families to assure needed supports and resources are available.
- Maintain consumer's medical record.
- Obtain and maintain clinical documentation and health records from other providers to ensure continuity of care.
- Participate in regular supervision with Clinical Coordinator and consult with other team members as needed.
- Attend staff meetings, case reviews, and training as required.
- Performs other work-related projects and duties as assigned.

- Valid Driver's License may be required depending on assignment.
- Certification in basic first aid and CPR, depending on area of assignment.
