Occupational Therapist - PACE/Elderplace

This hybrid position will be working at our Laurelhurst site and traveling to see patient in their homes in Northeast, Southeast and East Portland as well as intermittent coverage in Clackamas County.

Schedule : 32 hours/week on weekdays day shift

This position is responsible for OT evaluations, and development and implementation of the OT plan of care in accordance with PACE policies. The OT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The OT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The OT assumes responsibility for participants treated by the OTA and OT Aide.

Required Qualifications:

  • Bachelors Degree in Occupational Therapy

  • Upon hire: Oregon Occupational Therapist License

  • Within 30 days of hire: National Provider BLS - American Heart Association

Preferred Qualifications:

  • Master's Degree in Occupational Therapy, or

  • OTD (Occupational Therapy Doctorate)

  • 1 year experience working as an occupational therapist in a geriatric setting.
