Salary: $60 / Hour
The Physical Therapist is responsible for performing patient evaluations, as well as providing and documenting therapy services in accordance with the plan of care developed for each individual patient and physicians orders when applicable.
Minimum Requirements:
+ Current licensure as a Physical Therapist in the state of practice
+ Graduate of an accredited school of Physical Therapy
+ One (1) year of prior professional Physical Therapy experience preferred
+ Current CPR if applicable
+ TB Questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or state regulation)
+ Must meet all federal, state and local requirements
+ Must be at least 18 years of age

SKY REHAB, located in Bowling Green, KY is a 60 bed inpatient rehab providing rehabilitative services to people recovering from disabilities caused by injuries or illnesses. We help our patients regain their physical or cognitive abilities so they can return home at their highest level of independence. As a smaller, specialized hospital, we offer an opportunity to work in a fast-paced and friendly work environment where you really get to know your patients and their families. SKY Rehab Hospital has been accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) for the following services and specialty programs: Inpatient Rehabilitation (Adults, Adolescents, and Children), Brain Injury Specialty Program (Adults), and Stroke Specialty Program (Adults). Our hospital is also accredited by The Joint Commission.
Responsibilities
Responsible for providing and directing Physical Therapy services to patients, including assessment, treatment, program planning and implementation, related documentation, and communication. Functions under a physicians order.
Required Skills:
+ Current, valid, and active license to practice as a Physical Therapist in the state of employment required.
+ Current BLS certification from a Vibra-approved vendor required.
Additional Qualifications/Skills :
+ One (1) year of experience preferred. Ability to project a professional image.
+ Previous Hospital experience preferred
+ Knowledge of regulatory standards and compliance requirements.
+ Strong organizational, prioritizing and analytical skills.
+ Ability to make independent decisions when circumstances warrant.
+ Working knowledge of computer and software applications used in job functions.
+ Freedom from illegal use of and effects of use of drugs and alcohol in the workplace.
