? Home Health Occupational Therapist (OT) - Day Shift
? Location: Albuquerque, NM
? Shift: Monday-Friday, 8:00 AM - 5:00 PM
? Guaranteed Hours: 40 per week
? No weekends, holidays, or on-call required

Job Summary:

We are seeking a skilled and compassionate Occupational Therapist to join our Home Health team in Albuquerque. This OT will provide comprehensive, patient-centered care in the home setting, helping individuals regain independence and improve quality of life through tailored treatment plans and caregiver education.

Key Responsibilities:

Evaluate patients and develop customized treatment plans for physical, developmental, or functional impairments

Implement and continuously reassess therapy plans to optimize patient independence in daily living activities

Provide hands-on care in patients' homes, including written and verbal instructions for families and caregivers

Recommend, order, and educate on adaptive equipment to support safe function in the home

Maintain timely, accurate documentation of evaluations, treatment, and communication by end of each day (including admissions)

Collaborate with physicians, advanced practice clinicians (APCs), and interdisciplinary team members

Supervise Certified Occupational Therapy Assistants (COTAs) and instruct Home Health Aides/CNAs on program adherence

Participate in team conferences, in-services, quality improvement activities, and special projects

Advocate for patients and help coordinate community resources to support self-management

Qualifications:

Valid Occupational Therapist license in the state of New Mexico (or eligible)

BLS Certification required

At least 1 year of occupational therapy experience, preferably in a home health or outpatient setting

Excellent interpersonal, organizational, and communication skills

Ability to work independently in the field with minimal supervision

Must have reliable transportation and valid driver's license
