I. Position Summary:

Provides Speech-Language Pathology services in the following sites of service, as assigned and indicated, for Long Term Chronic Hospital, Out-Patient Therapy, Rehabilitation Services Unit and HSL Homecare. Responsible for diagnosis and therapeutic services for patients who demonstrate disorders of communication, cognition, and swallowing. Serve as a member of the interdisciplinary team to manage patient care. Act as a clinical resource for staff, patients, families and greater community in the areas of communication, cognition, and swallowing. This can include both internal and external presentations. Provides clinical instruction/supervision of SLP graduate externs. Participates actively in regular quality and process improvement projects in collaboration with Rehabilitation Manager, or clinical supervisor. Responsible for independently managing caseload assignments in order to effectively meet the needs of the patient while working within a set productivity expectation. Actively participates in both internal and external continuing education activities and shares knowledge with appropriate team.

Core Competencies:

  • Diagnosis and evaluation of communication, cognitive-linguistic, and swallowing disorders.
  • Establish a plan of care to address identified impairments. Set objective and measureable short and long term treatment goals.
  • Development of appropriate plan of care given patients goals, abilities, and prognosis for functional improvement.
  • Use evidence-based treatment methods, continuously assessing patients' response and modifying/progressing plan of care accordingly.
  • Provide objective, precise, and timely documentation of patient care, patient/family education, and information regarding charges for service.
  • Participate in regular professional development activities through participation in educational programs, review of current literature, in-service meetings, and workshops.
  • Share knowledge obtained in all continuing education activities with SLP team.

Position Responsibilities:

Core Functions:

  • Evaluate and review pertinent data relating to overall treatment planning and evaluation.
  • Administer standard evaluations and elicit information regarding existing skills and capacities.
  • Administer diagnostic testing procedures to determine the extent of the patient's impairment and the prognosis for improvement.
  • Organize and present educational information to caregiver/families on an individual basis.
  • Participate in the preparation and maintenance of departmental reports.
  • Responsible for maintaining a clean work environment
  • Responds to assist with responding to call bells and patient requests
  • Independently able to set personal caseload and schedule to meet the needs of the individual patients while considering the needs of the department and the team all within set productivity expectations.
  • Serve as an education resource for patients, families, interdisciplinary team members and greater community for all communication, cognitive-linguistic and swallowing disorders.
  • Demonstrates proficiency with documentation in the electronic medical record
  • Maintains confidentiality of patient and information.
  • Participates in quality improvement initiatives
  • Follows all policies and procedures set forth by the organization and outlined in HRC's policy manuals and the employee handbook.

  • Protect patient confidentiality by promoting professional staff communication according to HIPPA standards.

Technical Skills/Documentation

  • Demonstrates competence with data entry and access to the electronic medical record.
  • Maintains documentation in accordance with professional standards and guidelines, such as, but not limited to, evaluation results, individualized treatment plans, and progress reports.
  • Uses the computer as trained to access emails and other online resources.

Leadership/Development

  • Serves as a resource/mentor to the team, new employees, and students.
  • Is an active participant in shared governance (committees, tasks force etc.) as designated.
  • Seeks opportunity for professional development by attending internal and external trainings, educational programs and conferences.

Safety

  • Practices in accordance with established standards of care for safety of patient, self and co-workers.
  • Identifies and reports basic safety issues or problems (ex. Spills) and takes action whenever appropriate.
  • Identifies and reports errors/potential errors according to policy.
  • Uses equipment as trained and according to organizational policy.
  • Identifies malfunctioning equipment, removes from use, and reports according to policy.
  • Adheres to all infection control policies.
  • Promotes the health and safety of all by following established infection control procedures (i.e., hand washing, use of personal protective equipment).

Qualifications

  • Education level required: Master's degree in Speech Pathology.
  • Licensure/Board Certification: ASHA-Certificate of Clinical Competence,
  • Follows all policies and procedures set forth by the organization and
  • Must have reliable transportation and a current driver's license
