Physical Therapist

• Deliver skilled physical therapy services in inpatient, outpatient, and home health settings
• Create and adjust individualized care plans to meet functional and mobility goals
• Educate patients and families on exercise programs, safety, and rehabilitation strategies
• Document evaluations, treatments, and progress according to regulatory and organizational standards
• Collaborate closely with nurses, physicians, and other therapists to ensure coordinated, high-quality care
• Participate in case conferences and discharge planning
• Help supervise PTAs and assist with training and mentoring if needed

• Degree in Physical Therapy from an accredited program
• Active South Dakota PT license (or ability to obtain)
• Valid driver's license and reliable transportation for home visits
• Current BLS certification
• Prior experience in any of the following preferred: home health, outpatient ortho, or acute rehab
• Strong interpersonal and time-management skills
