RN - Case Manager

We are a reputable health services organization and have an opportunity available for a Registered Nurse Case Manager! The RN Case Manager is responsible for the development, implementation and evaluation of patient care plans. They will also coordinate all aspects of the care of individual patients and ensure proper utilization of services and resources. To that end, they will work closely with the Health Center Administrator and Medical Director to ensure the provision of quality care services. Please note you would be expected to travel to all of our health centers (Midtown, Harlem, Brooklyn, and Queens) at an ad-hoc basis.

Duties and Responsibilities:

1. Identifies patient's needs, current services and available resources
2. Develops, implements and evaluates individualized patient care plans
3. Documents patient's case management plans and on-going health care activities
4. Verified and evaluates intervention and treatment plans to ensure that they meet the patient's needs and goals
5. Educates and communicates healthcare plans to patients, family/caregivers
6. Identifies and addresses healthcare risk factors and/or obstacles to care
7. Perform other duties as assigned

Education and Experience

1. Licensed as a Registered Nurse in New York State
2. 7 years of nursing experience; Case Management experience preferred
3. Current BCLS and Infection Control Certification
