Job Description:
Provides individualized physical therapy including evaluation of the patient, conception and delivery of the plan of care, follow-up evaluations and discharge planning in accordance with state law and APTA guidelines to maximize the patient's progress toward achieving functional goals. Completes accurate and timely documentaiton of all care provided. Enters charges for services as described by department policy. Treats patients in the acute care, LTC and outpatient setting. Assists in performance improvement activities, safety monitoring and professional development. Provides services to all age groups.
Minimum Education:
- Bachelors, Masters or Doctorate degree in Physical Therapy from an accredited program by the American Physical Therapy Association (APTA) Commission on Accreditation in Physical Therapy Education.
