Job Profile Summary: The Interfacility Transport RCPhas an intuitive grasp of clinical situations and demonstratescomprehensive knowledge and experience. S/he is a recognizedresource, leader, and problem solver. S/he seeks out challengingpatients and shares expertise with others. The InterfacilityTransport RCP is an experienced RCP that provides comprehensiverespiratory care to the critically ill/injured patient (neonatal,pediatric, and/or adult with a pediatric chronic disease) in themobile environment of air and ground transports. Through the use ofexpanded skills, the Transport RCP is responsible for the accurateassessment, planning, implementation of and evaluation ofrespiratory care for assigned patient under the direction of theTransport Medical Control Physician. The transport team compositionconsist of RN/RCP and is capable of multi-mode transports utilizingground, rotor, and fixed wing. The Transport RCP is alsoresponsible for performing job duties that are consist with theBOE.Major Responsibilities: Patient Care Activitieso Obtainspertinent maternal, neonatal, and pediatric histories with emphasison risk factors and implications for problems and incorporates thatin plan of care.o Performs pertinent physical examination relatedto respiratory practice.o Formulates differential diagnosis and aplan of care of existing and potential problems in collaborationwith Transport RN and Transport Medical Control Physician (TMCP).oRecognizes and treats changes in clinical situations (includinglife threatening conditions) and implements changes to thetransport plan of care based on Standing Transport Protocols andTMCP Exercises independent judgment in situations when suddenclinical changes occur and understand and implements necessaryinterventions and their expected outcomes based on TransportProtocols and TMCP recommendationso Performs necessary diagnosticand therapeutic procedures as indicated for identification andmanagement of problems as indicated by protocols and/or TMCPrecommendations, including but not limited to (Patientstabilization, Airway management, needle aspiration of pleuralspace, interpretation of routine laboratory and radiographic data,intraosseous insertion, spinal immobilization, trauma primary andsecondary assessment/management, blood drawing, insertion ofperipheral IVs and all other procedures as trained).o Providesclinical management for resuscitation, with independent manner perprotocol, of the neonatal, pediatric, and/or adolescent patient,including necessary pharmacological support during interfacilitytransports.o Provides ongoing monitoring, assessment, andappropriate interventions in accordance with standing protocols andorders while in transit, at referring and receiving facilities anddocuments accurately on all transport documentation according todepartment guidelines and roles models this to other team members.oDemonstrates expertise in respiratory practice and criticallyevaluates own decision-making and judgments.o Serves as an expertwith ability to care for all types of neonatal and pediatricpatients (and adults with pediatric diagnosis/medical history), atan expert level, in the mobile environment (ground/rotor/fixedwing). Assuring safety and quality of careo Wears appropriateuniform and boots according to policy and role models to peers.oObtains appropriate transport consents including but not limited toconsent for treatment and transfer and privacy consent and ensuresunderstanding of information to guardian prior to obtainingconsents.o Completes independent verification during interfacilitytransfer for the RN on required medications (acting as RN'swingman)o Takes a leadership role in achieving unit compliance withTransport Program accreditation for CAMTS and actively participatesin the Quality and Safety plan of department.o Consistentlyutilizes risk assessment/reduction strategies and role models thisbehavior to coworkers to include but not limited to: fatiguemanagement assessment, ground risk assessment, post transportdebriefs and predeparture checklist and reports identified risk tothe leadership on a real time basis.o Communicates informationabout the patient's working diagnosis, plan of management, andprognosis to the referring physician, guardian, and othermanagement of the health care team and provides a concise andthrough hand-over and transfers care of patient to the receivinghospital staff.o Assure medications and supplies are stockedaccording to guidelines and are not expired and role models thisbehavior to coworkers.o Administers all inhalation medications asprescribed by TMCP (standing orders/protocols) and according topolicy and procedure.o Implements strategies to reduce patient andcrew risk and increase safety in the transport environmentincluding but not limited to the use of securement devices,securing equipment, and the use of lights and sirens Operation oftransport equipment/vehicle procedureso Demonstrates expertknowledge and operation of all transport equipment within scope andis able to troubleshoot issue on a real-time basis in order toprovide the safest care for the Maintains all transport equipmentin a state of readiness by checking all equipment, per departmentalguidelines, at the beginning of each shift and prn. Specialattention to detail needs to be made noting any discrepancies andtroubleshoots as needed. Role models this behavior to ensurecompliance with others on the team to maintain a culture ofsafety.o Participates in the ongoing evaluation of transportequipment needs and provides detailed feedback to ensure evaluationwas completed in a comprehensive manner.o Reports and documentstransport equipment malfunctions/errors to biomedical engineering,transport leadership and in appropriate patient safety eventreporting.o Demonstrates knowledge of ground, rotor and fixed wingaircraft operating procedures and safety practices. Transport TeamCommunication Centero Facilities and contacts consultants asdirected by TMCP.o Ensures appropriate documentation of transportrequest is completed with accuracy.o Coordinates initial transportrequest information from the referring facility unit clerk andbegin request in Central Logic.o Maintains appropriate contact withthe transport team at required times to ensure team's where aboutsand ongoing safety of transport team and maintains information on aguardian/designee ride along and logs emergency contactinformationo Keeps up dated information on current weatherconditions in and around transport service area and notifiestransport leadership of any current weather Coordinates andconfirms bed placement at receiving facility or alternativeadmission facility as Coordinates appropriate mode of transportwith vendor staff and contacts/communicates information to AdvocateChildren's Hospital Transport Team.o Communicates ETA and status,on an ongoing basis, with both the referring facility and receivingfacilityo Verifies appropriate crew information sheets on file andinitiates PIAP (Post Incident Action Plan) when Maintains up todate information on bed availability, physician, teamcoverage/status, vehicle information-availability, and contactnumbers on an ongoing basis. Professional Development/Professionalaccountabilityo Participates in peer review of the transport teammembers by accurately completing post transport debriefs.o Earns aminimum of 15 contact hours per year related to transport medicineand/or population that is served by transport program.o Takes aproactive approach in seeking out own learning needs andexperiences in order to stay up to date and current based ondepartmental educational plan and requirements.o Demonstratescontinued professional growth through continuing education andreview of current literature pertaining to neonatal-pediatrictransport as evidence by participates in at least 75% ofdepartmental journal clubs on a yearly basis and encourages peersto actively Takes a leadership role in the education of others atthe unit, house-wide and/or system level to provide a class and/orformal education event once a year.o Leads a transport case studyonce every 12 months AND/OR takes a leadership role in theeducation of others at the unit, site, system or at a ReferringHospital to provide a class and/or formal education event once ayear.o Actively participates in at least one unit committee and/orunit task force OR site/system committee and shares information andkeeps department up to date on current committee (task force)issues.o Serves as a preceptor in the training of future transportteam members and residents/students that are completing a transportrotation (required after member has been on transport team for over12 months)o Participates in at least 75% of department staffmeetings on an annual basis via in person or phone.Leadership/Research/Evidence Based Practiceo Initiates,participates, and assist with the implementation of change totransport policies, practices, procedures to enhance the quality ofcare that is delivered.o Identifies and/or participates inpotential/actual research studies and/or evidence base practiceprojects that are related to neonatal, pediatric, and transportwhen identified.o Serves as a resource or consultant to nursing,medical, and ancillary staff regarding neonatal and pediatrictransport and acts as an expert in complex cases.o Performs allrequired charge crew duties in a comprehensive manner, whenassigned, and communicates through the proper chain of command whenTakes a collaborative approach in assisting with medical proceduresand interfacility transports when requested and available and doesthis in a manner that is collegial, representing the transportdepartment in a positive manner.o Prepares for the safety huddle,when assigned, and communicates department information and followsup on any issues identifies according to guidelines established.oUtilizes resources and follows chain of command for disruptivebehavior that impedes the delivery of care/safety of patient and/orteam and proactively addresses difficult conversations on a peer topeer basis when appropriate.
