We have an immediate opening for a full-time Case Manager to support our Case Management Department SHIFT: Monday - Friday 8:00am - 4:30pm BASIC FUNCTION: The Case Manager (CM) coordinates inpatient rehabilitation care for a designated case load of patients and refers patients for post inpatient care CM is responsible for collaborating with all team members for identification of patient/family needs/goals, achieving economical utilization of services and planning discharge as well as follow-up care with the patient, family/caregiver/ and other facility The Case Managers serves as the IRF PAI Coordinator for his/her group of inpatients KNOWLEDGE/EXPERIENCE: Current Mississippi Registered Nurse License BSN, MS in Nursing and CRRN preferred Evidence of strong interpersonal skills, verbal and written communication skills Ability to physically assess and/or analyze patients with acute or rehabilitative requirements Demonstrates proficiency with utilizing standards of care, competency with nursing process and FIM scoring Knowledge of third-party reimbursement systems and utilization of referral services or ability to acquire knowledge Willingness to work flexible hours WE OFFER: Competitive pay A comprehensive, flexible, benefit package to all full-time employees (32 hours plus per week) Medical and dental coverage beginning the first of the month following 30 days of employment 403b Retirement Life Insurance METHODIST REHABILITATION CENTER 1350 E.
