Speech Therapist (Flex)

Therapists will ensure that they communicate, educate, and advocate while performing therapy in a fiscally responsible manner.Duties:Evaluates, plans, and provides speech therapy services for assigned hospice patients as prescribed by physicianImplements clinical services at home, including:Screening hearing abilityEvaluating communication performancePlanning and administering speech/language therapy programsCounseling patients and their families regarding communication performance and disordersMonitoring patient progress in therapy programProvide written reports of evaluation results, therapy progressConsult as needed with the assigned RN Case Manager to evaluate the speech/language therapy component of the Hospice Plan of CareConsult with the RN Case Manager to coordinate procurement of appropriate equipment for the patients use.Documents speech therapy interventions, evaluations, and observations in the hospice clinical record in a timely mannerAt minimum, weekly consultation with RN Case Manager to evaluate status of patients assigned for therapyOther duties as assigned by supervisorCareFirstNY encourages applicants to apply who may be seeking a non-traditional schedule, reduced working hours, or flexibility with their hours! Skills/Qualifications:Must have a valid and current NY State Language and Speech Pathology License with one year experience preferred. Must also have a valid drivers license and ability to commute throughout Chemung, Steuben and Schuyler counties. Ideal candidates will have on-site, weekday availability within first 90 days of hire to orient to their position.
