Job Summary and Qualifications

The Case Manager RN participates in departmental operations through analysis of Case Management practices and outcomes. Monitors compliance with third party payer requirements, government regulations and JCAHO standards to support quality of care for patients and maximize reimbursement. Daily monitoring of appropriateness of transfers, admission status, and length of stay, and the appropriateness and timeliness of post-discharge/departure symptom management through access of community resources. Facilitates interdisciplinary team approach to meeting the broad spectrum of the patients healthcare needs. Appropriate interpersonal and communication skills. Evidence of analytical and decision-making skills.

Participates in Performance Improvement activities to assure continuous Quality Improvement. Maintains appropriate awareness when dealing with patients, visitors, co-workers and other customers. Promotes a supportive, team-work oriented environment. Fulfills departmental responsibilities with the long-range plan, mission, vision and values of Horizon.

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• (RN) Registered Nurse license in the state of TN
• Associate Degree
• 3+ years of clinical nursing experience required
• Prior CM, UR and discharge planning experience preferred
• BLS required within 30 days
