Job Description

Assists in the development, implementation, revision and reporting of a case management system. Ensures coordination of patient care while working with all appropriate members of the healthcare team. Will perform a variety of high-level patient care activities including admission assessment, screening for appropriateness, assisting in the development of a treatment plan to address physical and psychological needs, coordinating services and discharge planning, and patient/family teaching. Verifies insurance coverage and obtains authorization and/or re-authorization for treatment as needed.

Education: Two-Year Associate Degree.

Experience: One-year prior experience is preferred.
