About Bridge to Home

Bridge to Home (BTH) is a new innovative program developed by NYC Health + Hospitals that will provide treatment, social and professional skill building, and temporary housing to patients with serious mental illness (SMI) who are ready for discharge from the hospital but do not have immediate access to stable housing.

In this voluntary program, BTH will offer its guests onsite ambulatory behavioral health treatment and comprehensive support in a home-like environment. The program aims to keep individuals discharged from the hospital on a path towards sustained success, with the goal of reducing emergency room visits and inpatient hospitalizations, decreasing street homelessness and reliance on shelters, and ultimately moving into permanent housing. Program guests will stay at the home-like environment until they can be transitioned to permanent supportive housing and treatment for a period of up to 12 months.

The Occupational Therapist uses purposeful activities or interventions to maximize the independence and the maintenance of health of any individual who is limited by a physical injury or illness, a cognitive impairment, a psychosocial dysfunction, a mental illness, a developmental or learning disability, or an adverse environmental condition. The occupational therapist uses skilled observation or the administration and interpretation of standardized or non-standardized tests and measurements to identify areas for occupational therapy services. Utilizing a trauma-informed and person-centered approach, the occupational therapist enhances the client's ability to live independently while managing their mental health, substance recovery, and preventive healthcare needs.

Core Responsibilities:

Provides preventive care and monitoring chronic health conditions for clients in a supportive environment.

Meets with guest onsite and throughout the building

Conducts health assessments, facilitates community and social integration, provides treatment, and education in consultation with the individual, family, and members of the treatment team.

Provides for the development of: sensory-motor, perceptual, or neuromuscular functioning; range of motion; or emotional, motivational, cognitive, or psychosocial components of performance.

Develops and implement individualized care plans & interventions that promote independent living skills, work readiness or educational performance, leisure skills, and social engagement, and community participation.

Facilitates therapeutic individual and group sessions focused on skill development, social interaction, instrumental activities of daily living, emotional regulation, and cognitive skills.

Demonstrates effective clinical decision-making, establishes treatment goals and completes all required documentation for patient care as required using appropriate medical and clinical terminology.

Participates in clinical education of fieldwork students as appropriate

Complies with NYC Health + Hospitals policies and procedures and all applicable local, state and national requirements and guidelines.

Performs other related duties as assigned.

Minimum Qualifications

1. Valid license and current registration to practice as an Occupational Therapist issued by the New York State Education Department (NYSED).

Department Preferences

Experience working with people with a serious mental illness and substance use

Experience working in a housing or shelter setting or community-based setting

Bilingual in Spanish
