Job Summary:
As a Care Coordinator, you will be responsible for:
Developing, monitoring, and evaluating home health care plans
Scheduling agency aides and ensuring shifts are covered, even on short notice
Managing a caseload of 100+ patients with a strong emphasis on retention
Matching clients with the most appropriate caregivers based on their needs
Communicating with referral sources and case managers to ensure excellent service
Handling caregiver and client concerns with professionalism and care
Reviewing care plans and addressing overutilization or staffing issues
Contributing to team goals and company mission
Qualifications:
High school diploma or equivalent (required)
Minimum of 2 years experience in Home Care or Service Coordination
Experience using HHA Exchange and EVV systems (preferred)
Proven ability to manage confidential information
Strong communication and customer service skills
Proficiency in scheduling and computer applications
Comfortable with closing and lead follow-ups
Highly organized and detail-oriented
