Responsibilities

As the Director of Rehab II (DOR II) , you will be responsible for overseeing patient care in an assigned host skilled nursing facility , in a manner consistent with your permitted scope of practice and in accordance with federal, state, and local regulations. The DOR II is responsible for the supervision of clinical programs ; organization and implementation of comprehensive therapeutic programs ; effective involvement in the hiring, training, and management of clinical personnel ; and overs ight of fiscal matters related to rehabilitation programs.

Consistent with your clinical scope of practice, the DOR II’s essential duties include the following:

  • Lead the on-site EmpowerMe team in delivering high-quality healthcare services that are person- directed and which produce outcomes in line with company goals and objectives
  • Perform rehabilitation services with residents 45% or less of the time.
  • Provide weekend scheduling for staff, as well as nighttime and early staffing needs for the facility.
  • Implement clinical programs that are focused on the needs of the residents living in a skilled nursing facility
  • Identify residents who may benefit from services through routine screenings, communication with care staff, and monitoring of electronic medical record(s)
  • Treat patients and their families with respect and dignity; identify and address psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families
  • Coordinate schedules of all services in the host community to optimize resident participation and productivity
  • Collaborate with host facility to ensure timely payor verification, assessment data reporting, care plan recommendations, and discharge planning
  • Comply with HIPAA (Health Insurance Portability and Accountability Act) and PHI (Protected Health Information) guidelines, and always protect confidentiality
  • Supervise a n on-site EmpowerMe team for a rehab program
  • Support the team’s operational needs to ensure team cohesion and satisfaction
  • Schedule therapists and residents to maximize efficiency and improve outcomes
  • Seek ways to improve and promote quality while demonstrating accuracy and thoroughness
  • Assist with meeting productivity goals and expectations, as identified by the company
  • Communicate updates regularly to the team; assist with training as needed, and monitor completion of required training
  • Actively participate in all required meetings and training
  • May participate in interviewing and onboarding new team members
  • May provide input in formulating budget, assist in evaluating department performance versus budget controls, and take appropriate action to remain within budget guidelines
  • Demonstrate competence in routine rehabilitation department operations, SNF policy and regulations as they apply to the rehabilitation programs, Medicare, m anaged care reimbursement , and required documentation to assure reimbursement
  • Provide leadership in such a manner that represents fundamental servant leadership principles, including effective listening, empathy, awareness, and persuasiveness
  • Ensure that company standards and policies are well-maintained, and align oneself with the company vision for service quality and value creation
  • Act as liaison between company and host community
  • Conduct regular communication with Administrator, Director of Nursing, and MDS coordinator
  • Promote EmpowerMe service offerings and provide in-services to host community’s care staff regularly
  • Communicate professionally and clearly, and maintain a good working relationship with physicians, care staff, the administrative team, and interdisciplinary teams, as well as demonstrate flexibility to ensure patient/facility/customer needs are being met
  • Participate in tours and help prospective residents and family members understand how EmpowerMe services will address their specific needs
  • Perform other duties as assigned that are consistent with qualifications, professional practices, and ethical standards

Should an employee believe they are performing the above referenced administrative and managerial services (e.g., non-rehabilitation services) for less than 55% of their work week, they must immediately report this to their Area Director of Operations, and/or Regional Director of Operations.

Qualifications

  • Graduation from an approved/accredited APTA, AOTA, or ASHA program
  • For Speech - Language Pathologists , an M.A. or M.S. degree in Communicative Disorder/Speech-Language Pathology. A current C ertification of C linical C ompetence ( CCC ) is also recommended .
  • Current certificate and/or license in the state in which they are practicing
  • CPR certification required if applicable, per state and setting
  • Minimum of one year of experience in a skilled nursing facility preferred
  • A passion for working with older adults while providing the best care possible
  • A solid understanding of rehabilitation offerings; Physical, Occupational, and Speech Therapy for seniors; and a general knowledge of laws and restrictions regarding Skilled Nursing Facilities
  • Knowledge of current practices in therapy, with emphasis in geriatrics
  • Exceptional communication and interpersonal skills , including ability to manage, train, and inspire staff
  • A bility to make timely decisions using accurate judgment and reasoning skills utilizing complex or diverse information
  • The ability to work well with others and take direction from management
  • Initiative and willingness to go above and beyond to ensure the needs of the organization are met in accordance with the company’s core values

Computer Skills:

Proficiency in Internet browsers (e.g., Chrome, Edge, Fire f ox), as well as advanced knowledge of Microsoft Office programs , including Outlook, Excel, and Word. Experience in working with various electronic medical records and medical billing systems.

Work Environment & Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. An individual should possess the physical ability to lift/move 50lbs, maintain a stationary position, move freely, operate equipment, ascend/descend freely, position self to reach equipment above or below average standing height, and communicate with employees throughout the organization. While performing the duties of this job, the employee is regularly required to talk or hear . The employee frequently is required to stand; sit; stoop; walk; use hands and fingers to handle or feel; and reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
