Essential Duties and Responsibilities
+ Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients functional needs and adjusts care plans to ensure optimal patient outcomes. Functions in various areas, which may include inpatient, outpatient, nursing home, and home health.
+ Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
+ Conducts therapy practice within defined standards of care.
+ Directs the assistance of support staff to achieve patients functional goals.
+ Communicates effectively with other professional and support staff to achieve positive patient outcomes.
+ Practices hospital and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
+ Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
+ Makes appropriate referrals to other members of the rehabilitative team (Occupational Therapy and Speech Therapy).
+ Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.
+ Performs Clinical Instructor (CI) duties for students on clinical internships.
+ Oversees, develops, and helps implement the Quality Assurance and Performance Improvement (QAPI) for the assigned departments.
+ Practices and behaves in a professional, safe and ethical manner at all times.
+ Serves as a patient advocate.
+ Maintains friendly and courteous interpersonal communication skills and a positive service attitude.
+ Participates in performance improvement and educational activities. Completes all CEUs as required by respective licensure. If licensed, maintains an active and current license.
+ Works willingly and accepts assignments as given.
+ Attends all mandatory meetings and programs.
+ Participates in opportunities that enhance personal and professional growth.
+ Performs other duties as necessary or assigned.
+ Complies with the Corporate Compliance Policy and all laws, regulations, and Standards of Conduct relating to the position and agrees to report any suspected violations of law or Standards of Conduct.
+ Complies with all applicable state and federal regulations and JCAHO standards.
+ Maintains a safe work environment using safe work practices, utilizing standard precautions and complying with hazardous waste procedures at all times as described in St. Anthony's Safety Manual.
+ Maintains the confidentiality of patient, resident, employee and organizational information.

This job has no supervisory responsibilities, however, may provide training, education, and guidance to assigned Physical Therapy staff and/or students.
Carries out responsibilities in accordance with the department Director, organizations policies and applicable laws.
Qualifications

Education and/or Experience
+ Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
+ At least one year of experience as a physical therapy aide.
+ Current license from the state Board of Physical Therapy Examiners.
Position Requirements, Licenses, Certifications, Registrations
+ Ability to work with a variety of patients and medical professionals.
+ Valid driver license.
+ Completion of MVR prior to hire and annually with an Acceptable grading criteria.
+ Proof of insurance upon hire and annually for personal vehicles used for company business (recommended minimum of $100,000 for property damage and $300,000 for bodily injury).
+ Excellent communication and interpersonal skills.
+ Knowledge and skills to care for the physical and developmental needs of patients receiving services.
+ Must possess effective time management skills.
Language Skills
Ability to read, analyze and interpret technical journals, financial reports and legal documents. Ability to respond to common inquiries or complaints from customers, patients, families, regulatory agencies, or members of the community. Ability to effectively present information to management, the public and/or board of directors.
