Provides case management services to children living with Autism. Caseload will vary based on the severity of clients. Clients require weekly or more frequent case manager contact. Moderate or low intensity cases are defined as not at risk who need varying levels of intervention based on their diagnosis, treatment plan, and family situation. The position is responsible for individual and family interventions to secure appropriate community services and improve family functioning. Case managers will lead various groups appropriate to their expertise and experience. ESSENTIAL RESPONSIBILITIES
- Assist in wrap-around team as needed in developing treatment intervention to de-escalate in the least restrictive settings.
- Provides crisis intervention and support services to clients on the caseload.
- Appropriately uses available crisis services based upon established criteria.
- Provides documentation and requests for authorization as required to funding sources to assure the Clinic receives payment for all services rendered.
- Assists clientele in obtaining needed Polara Health services, entitlements or other services.
- Coordinates and assists treatment team in monitoring enrollment and utilization, client care, and client participation in other Polara Health services.
- Maintains clinical record per agency policy and procedures.
- Attends all team meetings, case reviews, staff meetings, supervisory sessions and in-service training as scheduled.
- Management of all aspects of treatment, including referrals to additional resources when appropriate.
- Service provision shall be conducted at the Clinic, the client’s home or other community sites as appropriate to the case while ensuring the staff safety and ability to conduct the service.
- Active participation in department efforts to assess and provide customer service to both payer and clients.
- Assisting in department strategic planning and implementation through participation in department staffing, staff meetings, training and other meetings as assigned.
- Active participation in agency affairs, including marketing, promotion of mental health services, development of programs, and other related duties as assigned.
- Participation in on-site trainings attending workshops and seminars, and conducting trainings based on workshops attended and/or expertise developed individually.
- Conducting necessary case management to ensure proper referral and follow-up on associated services.
- Participation and documentation of clinical supervision per Clinic policy.
- Performs other duties as assigned.
- 21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member’s scope of practice; or minimum 18 years old for select administrative positions
- Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
- Must possess a valid Driver License
- Minimum Bachelor's degree OR Minimum combination of education and experience to equal 4 years in behavioral health will be considered
- One year clinical experience OR Minimum combination of education and experience to equal 4 years in behavioral health will be considered
- Knowledge of Behavior plans, ADHD, RAD, Conduct Disorder, Crisis de-escalation, and wrap-around services
