Case Manager - Casey House

Niagara Falls. Pinnacle Community Services. Case Manager - Casey House. $19 - $20/hour.

POSITION OVERVIEW:

The Youth Services Case Manager is responsible for supporting the Residential Shelter team in Case Management in addition to demonstrating knowledge of the mission and services of the Agency.

QUALIFICATIONS/EXPERIENCE:

* Associates Degree in Human Services (or related field) and one year experience required

* At least one year experience working with runaway, homeless, and at-risk youth in community-based or residential settings

* Experience in crisis intervention, case management, or counseling in a residential setting is a plus

* Valid NYS Driver License and use of personal vehicle during work hours

* Proficiency in Microsoft applications (i.e. Word, Excel, PowerPoint, Outlook, etc.) and ability to use web-based databases

AGENCY EXPECTATIONS:

* Reports all suspected child abuse/neglect to NYS Hotline with supervisory approval

* Attends and participates in appropriate Agency staff trainings and meetings and program supervision

* Advocates for cultural diversity/sensitivity as appropriate

* Experience and knowledge on the use of computers, cell phones and a broad understanding of using the Internet and email in the workplace

* Effectively communicate with consumers, community partners and Agency staff including accurate documentation

* Participate actively in ongoing professional development

* Meets with Program Supervision on a regular, planned basis

JOB RESPONSIBILITIES:

* Provide ongoing crisis services, positive youth development, trauma informed care, and support for (RHY) runaway, homeless and at-risk youth in an emergency shelter

* Complete program prescreening interviews and intake paperwork for new residents

* Interview, develop, and implement treatment plans in cooperation with individual youth

* Provide direct case management services to residential and non-residential clients

* Assess the needs of youth and make referrals to appropriate community agencies and resources; advocate for youth in community to ensure service needs are being met/maintained

* Maintain caseload of youth in residence and complete 30, 60, 90 day follow-up reports for discharged youth

* Establish working relationships with referral sources and other related service agencies

* Maintain residential and non-residential client files in compliance with regulatory agencies and complete paperwork in a timely manner

* Keep records for evaluating and statistical reporting to funding agencies

* Review current practices to advocate for change when needed, particularly those needed by RHY

* Participates actively in RHY training and education

* Educate youth in shelter on various topics including but not limited to HIV, AIDS and other items as determined by program guidelines. Orient youth to group home including resident guidelines, fire safety, etc.

* Team with program staff in order to provide consistency of services

* Transport clients as appropriate

* Represent Agency and Program in the community; conduct school and community presentations for the purpose of educating youth about program services

* Provides outreach for Casey House services to youth, agencies, personnel, families, schools, etc.

* Attends and participates in appropriate Agency staff trainings and meetings
