We are seeking a skilled and compassionate Outpatient Physical Therapist to join our rehabilitation team. The therapist will be responsible for evaluating, planning, and implementing treatment programs for patients with musculoskeletal, neurological, and post-surgical conditions in an outpatient setting. The ideal candidate will be dedicated to improving patient outcomes and quality of life through evidence-based physical therapy techniques. Responsibilities: Evaluate patients' physical conditions and functional status through physical exams and diagnostic tests Develop individualized treatment plans based on clinical diagnosis, patient goals, and functional outcomes Implement appropriate therapeutic interventions including manual therapy, therapeutic exercise, modalities, and patient education Monitor and document patient progress, adjust treatment plans accordingly, and provide clear communication with patients and other healthcare providers Collaborate with physicians, case managers, and other rehab professionals to ensure coordinated and effective patient care Educate patients and caregivers on home exercise programs, injury prevention, and rehabilitation strategies Maintain accurate and timely documentation in accordance with clinic policies and payer requirements Participate in staff meetings, in-service training, and continuing education as required Ensure a safe, clean, and supportive clinical environment Qualifications: Doctorate or Master's Degree in Physical Therapy from an accredited program Current state licensure (or eligibility) as a Physical Therapist CPR/BLS certification (or willingness to obtain upon hire) Experience in an outpatient orthopedic or sports medicine setting preferred Excellent interpersonal, communication, and organizational skills Ability to work independently and as part of a multidisciplinary team
