The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. The NCSB is seeking a compassionate and driven Case Manager III to make a meaningful impact on the lives of clients serviced in the City of Norfolk. In this role, you will serve as the agency liaison to the local jail for the purpose of assessing individuals in the jail to determine the presence of mental illness or co-occurring disorders. Individuals determined to have a mental illness, or co-occurring disorders will be linked to the appropriate NCSB program. Departmental Hiring Salary Range: $46,589-$53,577
Essential Functions
Essential functions include but are not limited to:
+ Serves as the coordinator to the jail to receive all referrals and assign caseload to other jail diversion case managers.
+ Works with appropriate jail staff and pretrial case manager to identify and provide discharge planning/jail release for all eligible clients.
+ Linkage to a mental health or substance use provider in the community (CSB or private provider) that provides psychiatric treatment and/or substant use treatment.
+ Provides case management to address short-term clients' needs which may impact recidivism and/or admission to a local inpatient facility.
+ Serves as a liaison to the jail to conduct face to face assessment to coordinate discharge planning.
+ Links eligible consumers to appropriate NCSB Program and serves as the care coordinator until services placement has been secured.
+ Provides linkage for Behavioral health appointments, medication, reasonable housing options, and linkage to other community support services as appropriate.
+ Assists consumers with acquisition of resources for stabilization, to exist institution quickly, and stabilize in the community.
+ Provides practical counseling and education to guide consumers in problem solving strategies to promote harmonious community living, and to resolve critical situations.
+ Provides onsite clinical interventions/appointments with identified participants at least weekly.
+ Develops and maintains a resource list of services for consumers.
+ Maintains consumer's records in accordance with NCSB standards and State Regulations.
+ Performs other related duties as assigned.
Education/Experience
Bachelor's degree in a human services field.
Two years experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
Preferred Experience:
Two years of experience with individuals with serious mental illness and substance use disorders and who also have criminal justice issues and involvement.
Additional Information & Requirements
+ Valid Driver's License may be required depending on assignment.
+ Certification in basic first aid and CPR, depending on area of assignment.
+ Regular full-time and permanent part-time employees may receive paid holidays, vacation and sick leave, employer paid pension plan, basic life insurance, voluntary participation in medical and dental, Section 457 deferred compensation, long-term disability, optional life insurance for self, spouse and children, medical and dependent care reimbursement plans, access to membership in credit union and employer provided parking.
+ Special project/grant employees are generally eligible for the same benefits of regular full-time employees; however, retirement, life insurance and participation in health plans vary depending upon funding and authorization.
+ Temporary/seasonal and part-time employees are generally only eligible for employer provided parking and access to membership in credit union.
+ Retirement
If you are hired October 5, 2010, or after to a position with City of Norfolk Retirement System benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
+ The Tuition Assistance Program is established to encourage employees toward continued self-development and education. Permanent full-time and permanent part-time classified, unclassified permanent, full-time special projects, and constitutional employees who have completed six months continuous service will be eligible to apply. The applicants school of enrollment must be an accredited institution.
