FTE: 0.900000 Shift: Shift 1 Job Summary: Evaluates and treats all

referred occupational therapy patients according to physician\'s orders.

EXPERIENCE DESCRIPTION: A minimum of 2 years occupational therapy

experience is preferred. EDUCATION DESCRIPTION: Master\'s degree from an

accredited Occupational Therapy program is required (unless

grandfathered in under Bachelor\'s degree). LICENSURE DESCRIPTION:

Wisconsin state license or eligible as a Occupational Therapist is

required. If eligible, must obtain license within 3 months of hire.

American Heart Association (AHA) Basic Life Support for the Healthcare

Provider (BLS) or an AHA approved equivalent is required within 30 days
