US:WI:MANITOWOC at our HARBOR TOWN CAMPUS facility. This job is ON -

SITE. FTE: 0.900000 Shift: Shift 1 Schedule: Clinic hours are Monday -

Thursday 6 am -5:30pm and Friday 6 am - 4:30 pm Job Summary: Evaluates

and treats all referred Physical Therapy patients according to physician

orders. Home Care: Performs and supervises physical therapy programs in

the homes of HFM Home Health Care clients. Other information: Degree in

Physical Therapy required. Wisconsin licensed Physical Therapist or

eligible required. If eligible, must obtain Wisconsin license within 3

months of hire. At least one year experience in a medical setting

preferred. Good communication skills required. CPR certification is
